Not every failed slab needs replacing. Where the concrete itself is intact and the problem is that the ground under it moved, lifting the slab back to grade is often the cheaper and less disruptive answer. The method injects material through small holes drilled in the slab, filling the void underneath and raising the panel back toward its original elevation. It works well on settled panels, sunken approaches, and slabs that have dropped where fill was placed over a trench or a culvert. It does not work on a slab that has broken into pieces, because there is no longer a panel to lift. The judgement between lifting, patching, partial replacement, and full replacement is the whole value of the assessment, and getting it wrong in either direction is expensive: replacing a slab that only needed lifting wastes money, and lifting a slab that has structurally failed wastes the lift. If the panel is still in one piece, usually yes. Injection lifting fills the void under the slab and raises it back toward grade through small drilled ports, and it is considerably less disruptive than removal and replacement. The test is whether the concrete is sound: a panel with one or two tight cracks can generally be lifted, while a panel that has broken into several pieces has nothing left to lift as a unit.

The usual culprits around here are backfill over a utility trench that was never properly compacted, fill over a culvert crossing that has consolidated, and water running under the slab edge and carrying the sandy subgrade away with it. That last one is why a repair that does not address where the water goes tends to become the same repair again a few years later.
No, and this is worth saying plainly. Concrete shrinks as it cures and some cracking is expected in any slab; that is what control joints are for, to give the shrinkage somewhere planned to go. A tight crack with no elevation difference across it and no movement over time is a maintenance item, best sealed to keep water out. A crack that is widening, that has a step across it, or that runs through multiple panels is telling a different story and deserves a look at the ground rather than the concrete.
Surface scaling comes from the top layer of paste being weaker than the concrete underneath, and the causes trace back to how the slab was placed and cured: water worked into the surface during finishing, finishing done while bleed water was still on the slab, or curing stopped too early. It is a durability problem rather than a structural one, and while resurfacers exist, they bond to the same weak layer that failed, so their success depends entirely on how sound what is left underneath is.
On a sound slab, sealing joints and cracks is worthwhile, because it keeps water from getting under the slab where it does the damage that matters. Whole-surface sealers are a different question and depend on the finish: decorative work is normally sealed as part of the system, while a plain broom-finish driveway can go a long life without it. What sealing does not do is fix a slab that has moved, and it should never be sold as if it will.
