The Pipe Under a Driveway Decides How Long It Lasts

Away from the city grid, a very large share of Montgomery County driveways cross a bar ditch to reach the road, and that means a culvert. The pipe is not a detail. It carries the ditch flow under the drive, and when it is undersized, set at the wrong grade, or crushed by the machine that installed it, the ditch backs up, water runs across the driveway instead of under it, and the fill under the concrete washes out from beneath the approach. That is why the order of work is fixed on a crossing: the pipe is sized against the drainage the ditch carries, it goes in and gets set to grade on a firm bedding, the fill over it is compacted in thin lifts, and only then is the approach formed to suit what is underneath. Reversing any part of that sequence produces the same result, which is an approach that has to come out. It also means the crossing is a distinct piece of work with its own cost and its own timing, sitting alongside the flatwork rather than inside it.

The pipe has to be in the ground and set to grade, and the fill over it has to be compacted. Those two steps decide almost everything about how the finished approach behaves, and neither can be done properly once concrete is sitting on top. It also means the crossing has a lead time of its own that belongs in the schedule rather than being discovered halfway through, and that utilities under the ditch line get located before anything is dug rather than after.
That is a drainage question rather than a preference, and it depends on how much water the ditch carries and what sits upstream of the crossing. Sizing it against the ditch is the whole point: a pipe chosen because it was the diameter on the yard at the supply store either backs the ditch up or costs more than it needed to. Get the size settled before buying pipe, because the wrong diameter is an expensive item to have leaning against a fence.
Two reasons, and they compound. The approach sits on fill placed over a pipe, and fill settles more than undisturbed ground unless it is compacted in thin lifts. It is also the part of the drive that every vehicle crosses at an angle, often loaded, and where the road edge flexes under traffic. Extra thickness across the crossing and properly compacted backfill over the pipe are the two things that keep it intact, and both are cheap while the ditch is open and expensive afterwards.
Often yes, but the existing pipe becomes the first question. A crossing that has been in the ground for twenty years under gravel may be crushed, silted up, or set at a grade that no longer matches the ditch, and none of that is visible until the gravel comes off. It is better to find out before the concrete is ordered than after the forms are set, so a look inside the pipe belongs early in the process.
It depends on the ditch and on how much fill sits over the pipe. A headwall or a sloped end holds the fill back at the pipe mouth, keeps the ditch flowing cleanly into it rather than eroding around it, and gives the approach edge something to bear against. Where the ditch is deep, steep-sided, or carries fast water after a storm, ends are worth having. Where they are used they should be sized to the pipe and to the ditch section rather than eyeballed, because an undersized end is a place for water to find its way around the crossing.
